Forms for Grant Seekers
Below you will find a listing of the Knott Foundation template forms required for submission with grant applications. Note: some of the forms are request specific.

Required for Step One: Letter of Inquiry
- Financial Analysis Form (Excel - Upload with Letter of Inquiry)
Required for Step Two: Full Grant Proposal
All applicants regardless of organization or request type must submit the following:
- Audit Insight Tool (Excel - Upload with application)
All applicants seeking funding for capital projects (including construction, renovation and/or property improvements) whose total project budget is $30,000 or more must submit the following:
- Construction Form (Word - Upload with application)
Note for capital projects: all construction and/or renovation work must be competitively bid, and copies of project bids, required permits, licenses, and signed contracts are necessary before funds are released.
All applicants seeking funding for development/fundraising (including salary support for development positions and all other operational development office costs) must submit the following:
- Development/Fundraising Form (Word - Upload with application)
All primary, secondary and special needs schools regardless of request type must submit the following:
- School Survey (Excel - Upload with application) This form will be emailed to you should you be invited to apply to the Foundation
All applicants seeking funding for technology (including hardware, software and/or peripherals such as SMART boards or projectors) must submit the following:
- Technology Form (Word - Upload with application)
